In order for you to enter information into from this website and allow your physician to see it at your next appointment, you will need to be given a username and a secret password.
Please contact your doctor or nurse in order to obtain a secure access code and password.
Before you begin to enter information from home, you will need to make sure that you have the file called Macromedia Flash installed on your computer. Please go to the link below to install this program if not on your machine already.

After you have successfully installed the software, you may enter The CATCH Study database, where you can describe how your arthritis affects you.

Once you have entered the study site you will see a screen called Clinical Analytics 4.0. Here you will type your firstname.lastname on the first line and then type your unique password on the second line. Once you have entered these correctly the information you have entered previously will be visible on the computer screen.
